FRED™ X is the next-generation FRED flow diverter featuring X Technology — a proprietary nanoscale surface treatment covalently bonded to the nitinol strands. The X Technology treatment is designed to reduce the material's thrombogenic potential at the blood-device interface without impeding the natural vessel wall healing response that is essential for aneurysm occlusion after flow diversion. In practical terms, this means the device may require less aggressive antiplatelet pretreatment in some patient profiles, though antiplatelet protocols remain physician-dependent. Beyond the surface treatment, FRED X introduces two procedural improvements: reduced delivery forces and reduced retraction forces relative to the original FRED, which improve navigability in tortuous anatomy and allow more controlled repositioning before final deployment. These refinements make FRED X preferable in vessels with significant cervical or intracranial tortuosity where the original FRED's delivery forces could cause the catheter to whip or the device to migrate during placement. FRED X extends the size range downward relative to original FRED, with implant diameters starting at 2.5 mm (for distal ICA and MCA M1 aneurysms) through 5.5 mm. The 2.5–3.0 mm System 21 variants are delivered via Headway 21; the 3.5–5.5 mm System 27 variants via Headway 27.

Derived from over 30 years of study and use at Terumo, X Technology is a proprietary, nanoscale surface treatment covalently bonded to the device for a permanent chemical change to the device surface. X Technology creates an optimized, protective hydration layer that is designed to allow for selective attachment to the device surface. FRED X is designed to reduce material thrombogenicity while also maintaining the natural vessel healing response. FRED X also features a reduction in delivery and retraction forces for easier tracking through tortuous anatomies.

Both systems use standard triaxial technique with a 6Fr guide catheter or long sheath, an intermediate catheter (SOFIA 6F or similar), and the delivery microcatheter.
- Is FRED™ X suitable for distal intracranial aneurysms?
- Yes. The FRED X System 21 (2.5–3.0 mm) extends flow diversion to smaller, more distal vessels that were previously outside the treatment range of larger-diameter flow diverters. This includes distal ICA, proximal MCA M1, and anterior communicating artery aneurysms that arise in vessels less than 3.0 mm in diameter.
